Code of Conduct Form

US Lacrosse Code of Conduct

In an effort to ensure that we provide a positive place for kids to play Lacrosse, we have posted a Code of Conduct form provided by US Lacrosse.  By reading and agreeing to the Code of Conduct we can ensure everyone will have a safe and positive experience with Lacrosse.

We will supply all players and their families with a Code of Conduct form that will need to be signed by the players and their guardians before their first practice.
